Emails/Texts:  The CCD program has many children enrolled.  Therefore, most communication is conducted electronically (text/email). By checking below, you are  agreeing to electronic communication, or stating that you will submit a letter expressing your desire for no electronic communication.

Correos electrónicos/textos: El programa de Educación Religiosa (CCD) tiene muchos niños matriculados.  Por lo tanto, la mayoría de las comunicaciones se llevan a cabo electrónicamente (texto/correo electrónico). Al comprobar a continuación,  usted acepta la comunicación electrónica o declara que presentará una carta que expresa su deseo de no recibir ninguna comunicación electrónica.

Medical Emergency/Emergencia Médica

I understand that reasonable precautions will be taken to safeguard the health and well being of the participants in this Religious Education program and that I will be notified as soon as possible in the event of an emergency. In the case of sickness or an accident, I authorize and consent the Director of Religious Education, or other associated volunteers of the Religious Education program to obtain medical care from a licensed physician, hospital, or medical clinic for my son/daughter in the event that I, or other legal guardian(s), cannot be reached. I hereby do release and forever discharge this Diocese and Parish from all manner of actions or claims which I or the child named above shall or may have for any reason, arising during my child’s attendance at Religious Education.

 Entiendo que se tomarán las precauciones necesarias para proteger la salud y el bienestar de los participantes en este programa de Educación Religiosa y que me notificará tan pronto como sea posible en caso de una emergencia. En el caso de enfermedad o de un accidente, yo autorizo y consiento el Director de Educación Religiosa, u otros voluntarios asociados del programa de Educación Religiosa para obtener atención médica de un médico, hospital o clínica médica con licencia para mi hijo / hija en el evento que yo, u otro tutor legal (s), no pueda ser localizado. Por la presente hago libero y para siempre descargo esta Diócesis y la Parroquia de toda clase de acciones o reclamaciones que yo o el niño mencionado supra o puede tener, por cualquier motivo, que surjan durante la asistencia de mi hijo en la Educación Religiosa.
